Uploading CE Credits to Application

New task: Track Recertification Credits - A screenshot of the application below shows this task.

Please see the task instructions on the application, which will link you to helpful resources.

This task will allow you to upload 1-2 files per activity for documentation, then fill out the required questions about each activity. More questions will appear when you upload a file for the next activity.

The questions about each activity include:

  1. Name of the activity

  2. Date completed

  3. Type of activity (Select 1 of 11 types)

  4. Number of CE Credits (based on the type of activity)

  5. Connection to Content Outline (Select one of four Domains that best connects to the Content Outline - this is just to help the reviewer better understand the activity, but it is ok if it's a guess)

  6. Additional Notes (Optional)

Renamed Task: E-File Cabinet (Optional): Previously: Upload Continuing Education Credits.

This task is not required for submission and is an optional tool for you. Consider it an "online file cabinet," where you can come back to download the uploaded documentation you've saved.

You can upload your files here if you are not yet ready to fill out the form about the activities.

If you've previously started a recertification application, you'll see the documentation you've already uploaded here. You will be required to upload it to the Track Recertification Credits task.
